In this Terms and Conditions, the applicant is referred to as “I” or “ME” and the creditor and its service agent(s) are referred to as “YOU”. Loans in Alabama, Arkansas, California, Delaware, Florida, Indiana, Kansas, Kentucky, Louisiana, Michigan, Minnesota, Mississippi, Montana, Nebraska, North Carolina, Oklahoma, Ohio, Rhode Island, South Carolina, Tennessee, Texas, Utah, Virginia, Washington, and Wisconsin are made by Capital Community Bank, member FDIC. LoanMart is a service agent for Capital Community Bank.

I certify that everything I have stated in this application, and on any additional documents I may be required to submit with this application, is correct. I understand that YOU will retain this application whether or not it is approved. By checking this box, I authorize YOU to check my credit and employment history and to answer questions that others may ask about my credit record. I agree to update credit information at your request if my financial condition changes.

I authorize LoanMart and Capital Community Bank to share any transaction history of financial products I have obtained through LoanMart or that are serviced by LoanMart for the purpose of evaluating me for credit.

Married Wisconsin Residents: No provision of any marital property agreement, unilateral statement, or court order applying to marital property will adversely affect Capital Community Bank’s interests unless Capital Community Bank, before the time credit is granted, is furnished with a copy of the agreement, statement, or court order, or has actual knowledge of the provision.
